We recently updated question #5 on our health screening survey (Have you or anyone in your household taken a COVID-19 test and are awaiting results?). Specifically, we added the words “or anyone in your household.”
Please note that you should answer no to this question if a person in your household has proactively taken a COVID-19 test in order to fulfill a requirement and is not symptomatic.
Examples of proactive tests include, but are not limited to, a mandatory work requirement, a mandatory school requirement, and as a requirement for a medical procedure. Additionally, if you or someone in your household has taken a COVID-19 test proactively, please inform your building nurse.
