AMS Families:
Below is information for the Pfizer COVID-19 vaccine distribution by Young’s Pharmacy scheduled for this Friday, May 14, from 4:30-7:30 p.m. for children ages 12-15 years old. Please note this event will take place in the AMS Cafeteria (not the gymnasium mentioned in the form below) and the second dose will also be at AMS.
Each participating child must attend with a parent/guardian. Participating children and their parent/guardian will need to enter the building through the PFA Entrance, located in the front of the building. There will be signs directing families where to go.
Parents/guardians must schedule an appointment for Friday using the following link: https://hipaa.jotform.com/Youngspharmacy/PFIZER1STAPMS05142021
Please also note that there are three pages to this form.
The first form is to register for Friday’s clinic.
The second form is to set an appointment for the return date of the second dose.
Finally, the third page of the form is demographic information required by New York State.
The forms on this site need to be filled out before arriving on Friday to help ensure a smooth clinic operation. We are aware that the form stated 16 years or older. Pleae disregard this and continue to sign up your 12-15-year-old child.
