If your child requires alternate transportation to a destination that is different from the primary home address listed on file due to circumstances such as childcare or a two-family household, please fill out the alternate transportation form by August 15. Please note that forms handed in after Aug. 15 may take up to two weeks to process.
This form can be found on the District website. Under the Services tab click on Transportation Services. The form is at the top of the page. The form can be faxed or emailed to 518-674-3629, davisk@apcsd.org or ryang@apcsd.org.
The alternate transportation form must be filled out and handed in every year even if the location is the same as in a prior year.
