NOTICE TO RESIDENTS OF THE AVERILL PARK CENTRAL SCHOOL DISTRICT
PRIVATE / PAROCHIAL TRANSPORTATION

Requests for transportation to private or parochial schools for the 2020-2021 school year must be completed and returned to the Averill Park School District no later than April 1, 2020. Separate request forms must be completed for each school.

State law requires submission of requests for transportation by April 1 of each year. If a child’s placement or acceptance at a particular school has not been finalized, return a completed request form in anticipation of attendance. If pertinent documentation is not received before the above deadline, the transportation service could be denied for our student(s). New residents must submit a written request immediately after establishing residence in the district.

The Request for Transportation can be obtained on the Averill Park CSD website. When on the site, go to Services, Transportation. The form is in the column on the right. Please fax, email or mail the completed form as soon as possible.
